Un nuovo trattamento per adulti affetti da osteoartrosi del ginocchio

Arthrosamid® è un gel idrofilo non assorbibile, non biodegradabile, iniettabile, trasparente, costituito da un nucleo di poliacrilammide saldamente legato e non biodegradabile (2,5%) e acqua apirogena (97,5%)

Arthrosamid® viene fornito come siringa preriempita, monouso, sterile, da 1 mL, sigillata con attacco Luer Lock e cappuccio per la punta. È destinato all'iniezione intra-articolare nell'articolazione del ginocchio con un ago sterile da 0,8 x 50 mm.

La figura 1 mostra la struttura tridimensionale dell'idrogel di poliacrilammide. La struttura a nido d'ape unito e uniforme fornisce una matrice per la crescita interna delle cellule.

Descrizione chimica della poliacrilammide

Come descritto sopra, l'idrogel Arthrosamid® è costituito per il 2,5% da sostanza secca di poliacrilammide reticolato e per il 97,5% da acqua apirogena. Durante la sintesi N,N,metilene-bis-acrilammide polimerizza con l'acrilammide creando così legami crociati tra le catene di poliacrilammide.

L'idrogel di poliacrilammide reticolato viene sintetizzato in un processo di polimerizzazione in cui unità ripetute di acrilammide sono collegate in una reazione a catena.

La reazione a catena prevede un principio di iniziatore redox, in cui un iniziatore di ammonio persolfato (AMPS) genera un radicale libero che aggiunge un monomero di acrilammide ottenendo un elettrone dal doppio legame nell'acrilammide, formando così un unico legame carbonio-carbonio con l'acrilammide . Ciò lascia un elettrone non condiviso per un'ulteriore aggiunta di monomeri di acrilammide, come illustrato nella Figura 2 a destra.

La ramificazione della dorsale carbonio-carbonio lineare della poliacrilammide si verificherà a causa della presenza di un agente reticolante, N,N-metilene-bis-acrilammide monomero (MBAM) come illustrato nella Figura 3.

Dopo la ramificazione delle catene di poliacrilammide della dorsale, si forma una rete tridimensionale di poliacrilammide reticolato, piuttosto che catene lineari di poliacrilammide non collegate.

I test di biocompatibilità sono stati eseguiti in conformità con ISO 10993-1 per i prodotti idrogel esistenti di Contura e i test considerati rilevanti per Arthrosamid® sono riassunti di seguito (Tab. 3) e descritti in dettaglio nel Biological Evaluation Report (BER) - Arthrosamid®.

Sulla base dei test effettuati, Arthrosamid® è considerato biocompatibile e adatto al contatto a lungo termine con il corpo.

Stabilità e durata di Arthrosamid®

La stabilità degli idrogel di poliacrilammide di Contura è ampiamente descritta e discussa nel Rapporto di valutazione biologica (BER) – Arthrosamid®, e di seguito è riportato un riepilogo dei test di stabilità che studiano la potenziale idrolisi, lo stress ossidativo e fisico su "Idrogel B" (Tab. 5). Come esaminato nel BER, la degradazione enzimatica della poliacrilammide è stata testata in vari sistemi di fermentazione enzimatica, ma non è stata osservata alcuna degradazione.

Si vede che l'idrogel di poliacrilammide è stabile e non si degrada nelle condizioni di test.

Arthrosamid®: stabilità - migrazione

Il potenziale di migrazione dell'idrogel di poliacrilammide è stato studiato in modo completo, come esaminato nel BER e nel rapporto “Evaluation of Arthrosamid® Migration and Degradation Potential After Intraarticular Administration”. In breve, in relazione all'iniezione intra-articolare, è stato osservato che piccole particelle (8 µm o meno) sono soggette a fagocitosi e fluiranno con il liquido sinoviale attraverso i vuoti delle giunzioni nell'epitelio sinoviale ed eventualmente saranno distribuite sistemicamente, mentre le particelle più grandi (> 8-17 µm) sono incapsulate nel tempo e rimangono immobilizzate all'interno dei tessuti molli indefinitamente. Come già accennato Arthrosamid®/”Hydrogel” è costituito da una matrice reticolata di catene di poliacrilammide. Il materiale finale, come viene iniettato, sostanzialmente non contiene componenti di piccole particelle e la dimensione più piccola delle particelle misurabili è risultata > 300 μm, che è ben al di sopra della dimensione delle particelle di un micron segnalata come fisiologicamente mobile.

Negli studi su conigli e cavalli sono stati osservati macrofagi e cellule giganti associate all'idrogel e non c'era evidenza di particelle di idrogel all'interno dei fagosomi in queste cellule (Christensen et al., 2016). Sono stati condotti ulteriori studi che hanno valutato i linfonodi drenanti e i tessuti distanti dai siti di iniezione dei tessuti molli, senza evidenza di idrogel nei linfonodi drenanti locali o negli organi distali (Charles River, 2011). Il potenziale di migrazione degli idrogel di poliacrilammide di Contura è ampiamente descritto e discusso nel BER, dove, sulla base della letteratura pubblicata e dei test specifici di Arthrosamid®, si conclude che l'idrogel di poliacrilammide rimarrà come impianto permanente nei tessuti sottosinoviali.

Arthrosamid®: prove cliniche e precliniche di iPAAG

Nei conigli e nei cavalli l'integrazione dell'idrogel è stata seguita fino a 2 anni dopo l'iniezione. Nei cavalli, 2 settimane dopo il trattamento l'idrogel è apparso come uno strato interno all'interno del rivestimento sinoviale mescolato a cellule sinoviali in proliferazione, simile all'istologia osservata in un modello di coniglio. A 1 mese, le cellule sinoviali si sono apparentemente riposizionate verso la superficie e a 3, 8 e 24 mesi è stato osservato un modello di integrazione simile (Christensen et al., 2016). L'idrogel era presente come zona integrata all'interno dell'interstizio sottosinoviale con una sottile rete di tessuti vascolarizzati e pochissime cellule infiammatorie.

È stato eseguito uno studio istopatologico prospettico sul tessuto prelevato dai pazienti durante l'artroplastica totale del ginocchio (TKA). I pazienti avevano ricevuto il trattamento con idrogel 5-33 mesi prima. Un modello istologico simile è stato osservato in tutti e sette i casi: si è scoperto che l'idrogel era integrato nella membrana sinoviale e le cellule del rivestimento sinoviale esterno erano entrate nel gel e avevano stabilito uno strato di rivestimento de novo. Un modello simile è stato descritto 9 mesi dopo la TKA in uno studio di caso (Christensen e Daugaard, 2016) .

Gli idrogel di Contura sono in commercio dal 2001 e sono stati condotti numerosi studi clinici con tempi di follow-up variabili per varie indicazioni. I dati a lungo termine sull'idrogel di poliacrilammide di Contura sono disponibili da uno studio di follow-up di 10 anni che comprendeva 104 pazienti con HIV, cui è stata iniettata una media di 6 ml di idrogel Aquamid® per il trattamento della lipoatrofia facciale. Al follow up (10 anni) nessun paziente presentava migrazione dell'idrogel e la maggior parte dei pazienti era "molto soddisfatta" (74,8%) o "soddisfatta" (23,4%) del risultato estetico (Negredo et al., 2015). L'aspetto stabile dell'idrogel è stato osservato anche in uno studio di follow-up di otto anni su 25 donne con incontinenza urinaria da stress che erano state trattate l'idrogel Bulkamid®, in cui tutte le pazienti presentavano depositi visibili di idrogel di poliacrilammide all'esame ecografico vaginale (Mouritsen et al., 2014).

Conclusione: stabilità e durata di Arthrosamid®

Come descritto in questa sezione e nelle precedenti, studi preclinici hanno dimostrato che Arthrosamid®/”Hydrogel” è biocompatibile, non assorbibile, non biodegradabile e non migrante (Bello et al., 2007; Charles River, 2011; Zarini et al., 2004). I dati clinici a lungo termine lo hanno confermato (Mouritsen et al., 2014; Negredo et al., 2015). È quindi ragionevole concludere che Arthrosamid® è stabile e sicuro per tutta la vita del dispositivo.

Controindicazioni

Arthrosamid® non deve essere somministrato:

  • Se è presente una malattia o un'infezione della pelle attiva nel sito di iniezione o nelle sue vicinanze
  • Se l'articolazione è infetta o gravemente infiammata
  • Se è presente un iniettabile intra-articolare degradabile come l'acido ialuronico, è necessario aspettarsi che venga assorbito secondo le informazioni del produttore per il prodotto specifico, prima dell'iniezione con Arthrosamid®
  • Se il paziente ha ricevuto in precedenza un trattamento con un diverso iniettabile/impianto non assorbibile
  • Se il paziente ha ricevuto un'artroplastica del ginocchio o ha materiale estraneo nel ginocchio
  • Se il paziente è stato sottoposto ad artroscopia del ginocchio negli ultimi 6 mesi
  • In pazienti emofilici o in pazienti in trattamento anticoagulante non controllato

Avvertenze

  • Arthrosamid® non deve essere iniettato per via intravascolare, extra-articolare o nel tessuto sinoviale o nella capsula
  • Non iniettare corticosteroidi insieme ad Arthrosamid®

Precauzioni richieste dal produttore

  • Arthrosamid® deve essere usato con cautela in pazienti con ad es. malattie autoimmuni e diabete non controllato, nonché in pazienti sottoposti a importanti interventi odontoiatrici o chirurgici
  • Prima dell'iniezione deve essere somministrato un trattamento antibiotico profilattico
  • Devono essere seguite le consuete precauzioni associate alle procedure articolari invasive
  • Nell'idrogel non devono essere iniettati farmaci o sostanze biologiche
  • La sicurezza e l'efficacia non sono state stabilite in pazienti di età inferiore a 18 anni, in donne in gravidanza o in allattamento o in pazienti con materiale estraneo impiantato nel ginocchio
  • Utilizzare Arthrosamid® solo se la confezione e i prodotti sono intatti e non danneggiati. Non risterilizzare Arthrosamid®.

**Il dosaggio raccomandato di 6 ml si basa sul volume totale di gel iniettato nello studio “proof-of-concept” (2 x 3 ml per la maggior parte (96%) dei pazienti) e sui dati dello studio CON-OA (5.4.4). Un'iniezione di 6 ml rispetto a 2 x 3 ml riduce il rischio di infezione e gli antibiotici profilattici vengono somministrati solo una volta.
